We, Seminole Animal Hospital Services, LLC d/b/a ShotVet, take your privacy seriously and use reasonable business practices to protect the confidential information of our clients. By using our website, online forms, online records, and prescription refills, you specifically agree and consent that we may use the Information and Communications in our sole discretion as long as such use is limited to our efforts to serve you and your animals. We expressly agree that we will never sell, rent, or otherwise use your Information and Communications in any other way. Should we undergo a reorganization, or sell our clinic or assets to a third party, you hereby consent that such Information and Communications may be transferred, but only if such reorganized entity or third party agrees to the same restrictions as are in this privacy section. All Information and Communications are intended to be confidential and between those who are specifically authorized. If you are not the intended recipient of the Information and Communications, you agree not to distribute or copy it in any way and to notify Seminole Animal Hospital Services, LLC d/b/a ShotVet, immediately.
Collection of Information
I hereby grant and convey to Seminole Animal Hospital Services, LLC d/b/a ShotVet and its authorized representatives, all right, title, and interest I may have in any and all photographs, video recordings, and other recording devices used during and around a visit to a Seminole Animal Hospital Services, LLC d/b/a ShotVet clinic. I further agree that Seminole Animal Hospital Services, LLC d/b/a ShotVet shall have the right to use such photographs and video recordings for any and all purposes, an unlimited number of times and in perpetuity by any and all means and media, now and hereafter created.
I understand that as my pet’s healthcare provider, Seminole Animal Hospital Services, LLC d/b/a ShotVet may remind me via email, snail mail, and/or text (standard text messaging rates may apply) when my pet(s) are due for refills, shots, and other medical-related treatments.
Financial Transaction & Pharmacy Distribution Disclosure
Due to the nature of our products, we are unable to accept returns or offer refunds for items once they have been dispensed.
Therefore, all sales for pharmacy items are final.

Refund Policy
Refunds will only be issued within 30 days of the initial purchase date. After this period, no refunds will be processed.
If a refund cannot be processed to the original payment method, store credit will be issued. Store credits are valid for 90 days
from the date of issue. Any store credits not used within this time period will be void.
Store credits can be transferred to a different pet or pet parent upon the verbal or written request of the client to whom the active store credit belongs.
Pharmacy Prescription (Rx) Policy
If a prescription (Rx) cannot be processed for any reason, the payment will automatically be converted to store credit.
The store credit for prescription (Rx) orders must be used within 90 days of the initial purchase, unless the pet parent requests a refund to the original payment method.
A patient doctor relationship is valid for 1 year from last visit. In order to dispense any prescriptions, a valid patient doctor relationship must be established and up to date.
YOUR PET IS EXAMINED TO DETERMINE THE APPROPRIATENESS OF THE IMMUNIZATIONS SELECTED. THIS IS NOT A FULL AND COMPLETE PHYSICAL EXAMINATION.
Event locations and times are subject to change with or without notification.
Prior to ordering products from Seminole Animal Hospital Services, LLC d/b/a ShotVet, all pets must attend one of our clinics and be examined to determine the appropriateness of any product ordered. You may only purchase products for the pet(s) that were examined at a Seminole Animal Hospital Services, LLC d/b/a ShotVet clinic.
A $25 research and consultation fee will apply for all prescription transfer requests outside of Seminole Animal Hospital Services, LLC d/b/a ShotVet.
Bundle and Discounted Pricing
Our packages are bundled and offered at a discounted price compared to individual item purchases.
If a refund is requested for an item that cannot be provided during service (e.g., specific vax not needed, blood can’t be drawn for test, etc), the total price of the package will be recalculated using the original individual prices of each item in the package.
The item that cannot be provided will be subtracted from the total, and any remaining balance, if applicable, will be issued as a
store credit.
Seminole Animal Hospital Services, LLC d/b/a ShotVet does not accept any competitors’ coupons/promo codes, or coupons/promo codes that did not come directly from Shotvet.com
A $6 bio fee is added to the total of all vaccination sales for the proper disposal of biohazard.
Florida Residents Only: THE PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.
Rabies Vaccination Terms & Tag Requirements
Rabies vaccinations with a 3-year expiry date are administered at the discretion of the on-site veterinarian and, according to local ordinances, require proof of the pet having previously received an initial rabies vaccination within the past year.
Residents located within select states and respective counties may incur additional fees related to their rabies tag. For specific
details, check local state and county guidelines.
Pet Health Disclosure & Vaccination Content
To the best of my knowledge, my pet(s) is healthy with no diagnosed allergies to vaccines and has had no recent occurrence of abnormal coughing, sneezing, vomiting, diarrhea, or weight loss. My pet has not bitten or scratched anyone in the last 10 days. I have disclosed all medications my pet is on. I acknowledge the fact that a vaccine reaction is possible. Should it happen that my pet becomes ill due to a vaccination, I will not hold the Veterinarian or Seminole Animal Hospital Services, LLC d/b/a ShotVet responsible, as reactions are highly individual immune responses that cannot be foreseen.
I understand that unforeseen side effects/allergic reactions may occur in any animal after the injection of a vaccine produced by major manufacturers of animal health products. Any side effects/ allergic reactions requiring further medical attention are at my own expense. Due to the nature of the clinic setting, free medical for vaccine-related side-effects and allergic reactions during clinic hours will be given, but is limited, and my pet may require further treatment at my expense. All other treatment(s) required, post-vaccinial events, will be at my own expense. I have been given the opportunity to ask any questions concerning this policy before signing below. Being aware of these facts, I give my permission to the Veterinarian to administer the vaccines and medications listed on this document.
A vaccine reaction is rare, yet potential side effects from vaccination include fatigue, muscle aches, stiffness, mild fever, or a small lump at the vaccine site. These side effects are usually temporary. Welts, facial swelling, vomiting, diarrhea, difficulty breathing, altered mental state, or shaking could be an allergic reaction. If any allergic reaction symptoms occur and progress beyond a period of 2-3 days, proceed immediately to the nearest animal emergency facility.
Booster vaccinations are required approximately 4 weeks after the administration of the first vaccine.
